Yes, vaping appears to increase the risk of gum disease, though not as much as smoking. UK and US dental research consistently shows higher rates of gum problems in vapers than non-users, but significantly lower rates than in smokers.
Gum disease has two main stages. Gingivitis is the early, reversible stage. Gums become red, swollen and bleed when brushing. Periodontitis is the advanced stage. The infection spreads to the bone supporting your teeth. If untreated, it leads to gum recession, loose teeth and eventually tooth loss.
Gum disease affects around 45% of UK adults to some degree. It is one of the most common chronic conditions in the country and one of the leading causes of tooth loss in adults.
Multiple studies have now looked at gum disease rates in vapers. The findings are reasonably consistent.
A 2020 study in the journal iScience found that 43% of vapers had gum disease or oral infections, compared to 73% of smokers and 28% of people who neither smoked nor vaped.
A 2024 systematic review found that vapers had a greater risk of periodontal disease than non-smokers but a lower risk than smokers. A separate study from NYU College of Dentistry in 2022 found that vapers had a distinct oral microbiome compared to both smokers and non-users, and that this altered mix of bacteria triggered different immune responses in the mouth.
The honest summary: vaping carries real gum disease risk. Less than smoking, more than nothing.

Less blood flow to the gums means less oxygen and fewer immune cells reaching tissue that needs to heal. Infections can take hold more easily and progress further before being noticed.

Use a soft-bristled brush at a 45-degree angle to the gum line. Two minutes minimum. Use fluoride toothpaste. The single biggest factor in preventing gum disease in anyone, vaper or not.
Floss or use interdental brushes daily
Removes plaque from between teeth where your toothbrush cannot reach. This is where most gum disease starts.
Drink plenty of water
Combats the dry mouth that vaping causes. Helps saliva flow, which is your mouth's natural defence.
See a dental hygienist every 6 months
Professional cleaning removes built-up plaque and tartar that home brushing cannot. Particularly important for vapers because of the higher baseline risk.
Use an alcohol-free mouthwash
Alcohol-based mouthwashes dry the mouth further. Look for ones with chlorhexidine or cetylpyridinium chloride, which target the bacteria that cause gum disease.
Lower your nicotine strength
Less nicotine means less vasoconstriction and better blood flow to the gums. Most vapers can step down 5mg or 10mg without significant cravings.
Tell your dentist you vape
They will check more carefully for early signs of gum disease and may recommend more frequent visits. Not for moral judgement, just for better care.
